CANCELED/MISSED
APPOINTMENTS:
A scheduled appointment means that time is reserved only for you.
If an appointment is missed or canceled with less than 24 hours
notice, the client will be billed according to the following schedule:
1st
missed session without 24 hr notice: half of scheduled
fee
Any session thereafter: full scheduled
fee
*First
consultation/session is free (up to 30 minutes either on the phone
or in person).
For in person free consultation: Please call within 24
hours if you need to cancel, otherwise the missed session will

Take 5S to Sorrento Valley Road
2. Go over the train tracks and go STRAIGHT
3. Make RIGHT into the first parking lot.
From
5 North:
1. Take
5N to the new byway (towards the 56)
2. Get
off at the Carmel Mountain Road Exit
3. Make a RIGHT onto Carmel Mountain Road
4.
Go to the end and make a LEFT onto Sorrento Valley Road. Go about
1.5 miles, past the train station. You will reach a big intersection.

Make a LEFT onto Sorrento Valley Blvd.
6. Make RIGHT into the first parking lot.

1. Take 805N to Mira Mesa Blvd

Turn RIGHT onto Sorrento Valley Road (if you turn left it becomes

all of these directions look for the yellow steps. There will
also be a red/white sign on the road for the MMA ACADEMY and Martial
Arts. Pull into the parking area by the Martial Arts sign and
enter in the front of the building.
FEES
First session
is always free (30 minute phone or in-person consultation): same
24-hour notice agreement applies.
The Therapy Hour
Fees are based on a "therapy hour", which is 50 minutes.
This allows the remaining time to receive payment and for me to
complete your session notes.
A "Double Session" is two combined 50-minute sessions,
totaling 100 minutes.
For
Individuals:
starting at $95 per hourly session (therapy hour 50 minutes)
Package Discounts:
$5 off per session if you purchase 5 upfront ($25 discount)
$10 off per session if you purchase 10 upfront ($100 discount)
*If you purchase a package and choose not to complete
your sessions, you will be refunded minus the difference of the
full session fee of the agreed upon hourly fee.

Most PPO and POS insurance taken as an out-of-network provider.
For
Families & Couples:
starting at $120 per hourly session
"Double sessions" are offered at a discounted rate of
$220 (for two 50-minute sessions, totalling 100 minutes)

$15 off per session if you purchase 10 upfront ($150 discount)

Insurance
is accepted as an Out-of-Network Provider:
What
does it mean to be an "Out-of-Network" provider?
If you choose to use your insurance, all fees are paid upfront
and then you are reimbursed by your insurance carrier. Depending
upon the carrier, reimbursements are usually received between
2-3 weeks after submission.
Please

TERMS ::CANCELED/MISSED APPOINTMENTS
Confidentiality:
All information between counselor and client is held strictly
confidential, unless:
1. The client authorizes release of information with his/her signature.
2. The client presents a physical danger to self.
3. The client presents a danger to others.
4. Child/elder abuse/neglect are suspected.
In the latter two cases, we are required by law to inform potential
victims and legal authorities so that protective measures can
be taken.
Confidentiality
via Telephone, E-mail, and Text Messaging:
Voicemail/Telephone: All messages and telephone exchanges
will be conducted with the utmost confidentiality.
E-mail
& Text-Messaging: Your therapist will ensure your privacy
with the utmost confidentiality, however due to the arbitrary
security of the internet and any type of electronic messaging
(i.e. text messages) please note: Any E-mails or Text
Messages are exchanged at your own risk.

Financial Terms:
The client is responsible for payment of psychotherapy services.
Unless otherwise negotiated with therapist payment of services
is due in full at the time of service. Payment arrangements should
be made prior to your first visit.
Fee per visit begin at $95 for individuals, $120 for couples/families.
This feel shall be paid in full at the time of appointment (see
above for Health Insurance information).Therapist reserves the
right to increase the fee with reasonable notification (at least
30 days).
